Your Right and Responsibilities as a Patient:
- Request and inspect copies of your dental records/X-rays and payment-treatment history (itemized statements), for a duplication fee
- Inform us of any changes in your personal information, such as a new address, telephone number, dental insurance carrier, or your medical history
- Notify us at least 24-hours in advance for re-scheduling or cancellation of appointments
